Service Description
* One on one consultation * Physical and mental assessment * Includes: Appetite Suppressant prescription, Weight tracking, consultation for diet/exercise, and supplies * Individualized weight loss plan to designed to help you loose weight and keep it off * Treatment plans include oral medications, and peptide injections to be used in conjunction with a healthy diet and regular exercise * Follow up visits are $45/month, we require minimum 3 months for continuity and to reach your goals * Most visits can be done via telehealth * Blood work can be done at your preferred location or we can come to you. * Not included with membership plans
Cancellation Policy
* Must book at least 4 hrs in advance. * Initial $ 25 Telemedicine consultation fee is due at appointment booking. If you proceed into treatment with us, we will credit your account. No show = no refund. * Cancel or Reschedule at least 24 hours before appointment time. Canceling appointment less than 24 hours before appointment time can result in forfeiting your consultation fee. * There is a $35 credit card return payment fee.
