HOW TO APPLY

We can help, our compassionate provider can help you qualify for your Medical Marijuana Card with a simple telehealth video visit. If you qualify, we will fill out the Healthcare Form
needed for your application. If we find that you don't qualify, we will give you a 100% refund.

Scan and upload your paperwork here and choose your dispensary.

You will receive an email to print your Medical Marijuana Card within 24-48 hours.

$100 for a 1-year ID card

Reduced fee Application: SSDI, SSI, and Veterans

Care givers:

$75 for 3 year ID card

$75 if applying separately for an already registered patient. Expiration date for caregiver and patient card are the same.

Currently, medical cannabis has two types of renewals:
-
Extension renewal- a yearly incremental renewal for patients that did not purchase a three-year card at the onset of registering and wish to continue with the Medical Cannabis Patient Program. No Physician Written Certification is required for this type of extension.

Full Recertification renewal- requires patients to have a new updated physician certification as well as additional documents. Be sure to read all renewal letters in their entirety.

QUALIFYING FOR A MMC CARD IN ILLINOIS
CONDITIONS THAT QUALIFY CONDITIONS

Be a resident of the State of Illinois at the time of application and remain a resident during participation in the program
-
Have a qualifying debilitating medical condition

Have a signed provider certification (unless you are a veteran receiving medical care at a VA facility)
-
Be at least 18 years of age
-
Not hold a school bus permit or Commercial Driver’s License (CDL)
-
Not be an active duty law enforcement officer, correctional officer, correctional probation officer, or firefighter.
